What are the options for drafting a Supported Decision-Making Agreement?
Use Our Agreement Builder: You can fill out a Supported Decision-Making Agreement (SDMA) through this website. If you would like to do this, go to the next page by clicking the button at the bottom of this page.
Look at Samples: You may also decide that you want to use SDM in a different way. What matters is that it works for the adult with developmental disabilities. You can find additional resources at the National Resource Center for Supported Decision-Making.
SDMAs should be tailored to each adult with developmental disabilities. We suggest that you use a form as a starting point. For example, you may want to add pictures if the adult with developmental disabilities is very visual. You will need to work with a lawyer if you are interested in preparing estate planning documents to accompany the SDMA.
Have Specific Questions?
To learn more about how to use SDM in Florida—including how to incorporate tools like the estate planning options discussed on this website—watch the webinar “Protecting the Right to Decide: Supported Decision-Making in Florida.” It is co-presented by Viviana Bonilla López, an attorney specialized in SDM, and Michael Lincoln-McCreight, the first person in Florida to terminate a guardianship using SDM. The presenters discuss Florida law and Michael’s personal experience.
