Letters of Guardian Advocate of the Person Only

Many counties require you to turn in a blank Letters of Guardian Advocate form.  Though it is not always required, we recommend that you complete this form so you have it as part of your petition packet if necessary.

The automated form is designed to generate the correct documents whether you are applying as single or co-guardian advocates.  You will only need to fill out the automated form one time regardless of the number of guardian advocates.


We'll ask you one question at a time and your answers will fill out the Letters of Guardian Advocate form.​  Then the form will be emailed to you.
