Oath of Guardian Advocate, Designation of Resident Agent and Acceptance

The Oath of Guardian Advocate form needs to be filled out by each person wanting to be a guardian advocate .

We'll ask you one question at a time and your answers will fill out the Oath of Guardian Advocate form.​  Then the form will be emailed to you.  If a second guardian advocate needs to fill out the form, we will send you back to this page to complete it for them too.

This form needs to be notarized before it is turned in to the county clerk of court.  Please do not sign this form until you are in front of a notary.
