Broward county requires all prospective guardian advocates to fill out the Mandatory Guardianship Investigation Checklist.

We'll ask you one question at a time and your answers will fill out the Mandatory Guardianship Investigation Checklist form.​  Then the form will be emailed to you.  If a second guardian advocate needs to fill out the form, we will send you back to this page to complete it for them too.​

** Please fill out and complete this form for each person that is applying to be a guardian advocate (maximum of two per petition).​
