Initial and Annual Guardianship Plans

Annual Guardianship Plan Document

You will also be required to fill out an Annual Guardianship Plan document unless it is specifically waived by the court.

Your county clerk can help you get the correct version of this document (visit the County Resources page for the Clerk's info).  Our recommendation is to get both this and the initial plan document from the clerk at the same time after the judge has signed the order.  Then you will already have a copy of it when the time comes to fill out the annual form.  
Please note - There are two possible methods your county may use to determine the filing date for the Annual Guardianship Plan document.  You will be given the correct filing date method in your required guardianship class.  If you are not given the correct filing date method in your class, please contact your county clerk.

  • Option 1 - Some counties require that the Annual Guardianship Plan document is turned in at the end of the fiscal year on April 1st.  
​
  • Option 2 - Some counties require that the Annual Guardianship Plan document is turned in within 90 days of the last day of the anniversary month that the judge signed the Letters of guardianship.  
    • ​If your county follows this option for the filing date, please see the calendar below to determine the date your Annual Plan document is due to the clerk.  
    • Please note there is a two month time period that you have to file your Annual Plan document each year.  We recommend that you try to get the document filed in the early to middle part of the period to avoid any last minute issues or complications.
