Initial Guardianship Plan Document

In most counties it is necessary to fill out an Initial Guardianship Plan document and give it to the court within 60 days of the judge signing the order.  

Please contact your Clerk of Court using the County Resources page.  They can provide you with a version of the Initial Guardianship Plan document that is valid for your county. 

Your county Clerk of Court can also provide assistance if you have any questions on how to fill out the form.
